This has been building for a while, it was bound to break eventually. Sans’s insecurities here are all things that have either been not-so-subtly implied to him (like here, here, here, here, or here) or outright said to his face, and he’s also still haunted by how he was used to try and get Papyrus to kill something and how his most recent attempt to protect him ended in total failure. It wears on you. Papyrus has known about his "condition" since they were babybones but had no idea how deep his feelings about it ran.

Papyrus has indeed had a scar on his neck since he had that neck brace. It may be hard to see at points but it’s there! He still has it in the future, even. And Sans did indeed tell Papyrus not to let Gaster get to him, haha. LOT OF BACKLINKS THIS ROUND
Sans is glowing a little bit of Papyrus's color there.
