There are only two exceptional cases of this headcanon in the story:
The first one was the time that Chara replaced Frisk and killed Sans. At that timeline, Sans never expected to face two humans both existing in the same body.
The second time was Frisk’s return(Sans almost thought it was Chara) Because, before that moment, Sans didn’t really believe he would see Frisk again.
By the way, the following headcanons will be more “Stand-in only” from number two since the related premises are not specifically addressed or displayed in the game.

) Or was there some kind of reset after chara killed sans?
