The logo to the 1993 Sega Genesis / Super Nintendo Entertainment System game "Pac-Attack" (パックアタック) by Namco.

GEN version found on:

Namco Anthology 2 (PS) (Japan) (1998)
Pac-Man Collection (GBA) (US/Europe) (2001)
Namco Museum (PS2) (US) (2001)
Pac-Man Collection (GBA) (Japan) (2002)
Namco Museum (GCN/Xbox) (US) (2002)
Namco Anthology 2 (PSN) (Japan) (2013)
Pac-Man Museum (Xbox 360/PS3/PC) (2014)
Pac-Man Collection (Wii U) (2014)

SNES version found on:

Wii Virtual Console (US/Europe) (2008)
Wii U Virtual Console (US/Europe) (2015)

Note: These ports of the Genesis version are not the original Sega Genesis game, but an updated version that originated from Namco Anthology 2. The updated version has some changes:

The music is somewhat higher quality and the sound effects have been remastered.

When Pac-Man eats a ghost, the ghost disappears when he contacts the ghost. In the original Sega Genesis version, as soon as Pac-Man reaches the block that the ghost is in it disappears.

An "appendix" mode was added to the puzzle mode that reduces the number of Pac-Mans available but uses specific formations for each level. Hold right when selecting the puzzle mode to access it.

The original Sega Genesis version had a sound test that could be accessed by inputting "SND" as a password. The updated version no longer has a sound test. The password "JFK," which goes directly to the ending screen, was also removed.

The Pac-Man Collection and Namco Museum ports skip the title screen and go straight to the main menu.
The GBA port also has edited music and no two player mode.

The version on Pac-Man World 2 is the arranged version from Namco Anthology 2, which does use a different logo. Here's what the original and arranged logos look like: [link]
