This is a What if idea, What if the Super Soldier Serum failed & turned Steve Rogers into the first Hulk, in this story, Steve Rogers became a disfigured monster that has wounds all over his body, Abraham Erskine regreted creating the serum at that point after what he had done to poor Steve Rogers who was screaming in pain as he was transforming, after that, the nazi spy still killed Erskine but Steve killed him before running away, the military still tried to create a serum better than Erskine's serum with it's uncontrolable efects, they also ended up by accident with a serum that killed alot of the black soldiers who ressived the serum, Isiah Bradley still survived and became this universe's Captain America, much to the americans' dislike (because this is the 40s) while Steve Rogers became what the public & governent call American Failure.
