Hakuo Mikayama was a gifted young physician in the employ of the wealthy and powerful. He was famous both for his innovative techniques and deep knowledge of traditional medicine. So it came to pass that he was summoned to a country noble's estate to care for a child who'd fallen ill. The strange disease swiftly spread to others in the household-- including their doctor. The only person to survive the initial stages of the infection was Hakuo, and the disease left him badly disfigured and prone to spells of weakness and terrible pain.

Hakuo fled the world he had known and took up residence in a remote mountain temple. To protect himself from local bandits (and to protect others from contracting his condition) he fashioned a fearsome mask from the broken guardian dog inside. There he began to research his condition, experimenting with exotic medicines to find a cure with curious but ultimately unsuccessful results. That was where Master Zenji came across him in his travels, and recognized the genius physician through his disguise. Without any fear or revulsion, the old swordmaster invited Hakuo live and work at his school, and Hakuo agreed.

Hakuo Mikayama acts as the doctor and all-purpose researcher for the school. He is no longer contagious, but his illness persists. He was a strong and handsome man before the disease, and where his appearance has suffered, his strength has doubled. His nerves have been deadened and his body is much more elastic, making him an impressive opponent in combat. Despite this, he is a very gentle and loyal man at heart.
