Actually this theme based on Vertex Theme

mondfried are you using Unity desktop?
If yes, This theme doesn't support unity :)
But if you need "index.theme" put this:

[Desktop Entry]
Type=X-GNOME-Metatheme
Name=Vertex-Green
Comment=A green dark theme based on Vertex
Encoding=UTF-8

[X-GNOME-Metatheme]
GtkTheme=Vertex-Green
MetacityTheme=Vertex-Green
IconTheme=gnome
CursorTheme= DMZ-Black
ButtonLayout=menu:minimize,maximize,close

thank you for your prompt reply. index.theme makes no difference, it won't show up in the appearance settings.

What version your GTK?
Make sure you've "murrine engine" and the latest GTK-3.22 up (On my test this theme working using GTK-3.24 too).
Also try add this theme first in your home dir (~/.themes) before you copy paste on /usr/share/themes/.
If you use GTK-3.24 we donn't need "index.theme" or "xfwm4" again because Xfce GTK-2.0 jump to integration with GTK-3.0. So....it will follow Gnome "CSS" file for window button layout :)
