Five years after the fall of Colonel Garret's militia, Oregon is trying to rise from its ashes. Deacon St. John and Sarah have settled with other survivors in a fortified camp, dreaming of lasting peace. But this calm is illusory...

When a new threat emerges: faster, more organized Freakers... and those capable of thinking, led by a secret faction. Led by former NERO agents, themselves infected by a mutated version of the virus, including James O'Brian. These intelligent infected seek to create a new world where "normal" humans would no longer have a place.

Deacon, torn between his desire to protect Sarah and the need to understand this mutation, delves back into the ruins of the old world. Along the way, he discovers that NERO left behind much more than experiments: active facilities, biological weapons... and a plan for extinction.

Joined by old allies and new faces, Deacon must face unprecedented horrors and make a heartbreaking choice: save what remains of humanity... or allow the emergence of a new species.
