Not long after leaving Qo'nos the Klingons where bound by their early space weaponry being very ineffective at longer ranges, partly due to the low technological level of the designs, early targeting computers, and the Klingon mindset of close in fighting, thus most fights where done within around 2 kilometres, and the qeylIS was used not as a heavy explosive, but as a boarding torpedo. While you could have a torpedo that missed at the longer ranges, thus these where usually used as the battle drew closer, or where used on rapid engagement craft that could only carry two or three, later models came with some self guidance but this was near the tail end when lighter and faster ships became the norm, and weapon ranges started entering the hundreds of kilometres.
