Thanks and I'm glad if I can help. I scan it at least 300dpi b&w photo, then you need to fix it in Photoshop to look decent. First fix the levels in Image Adjustment. Second, clear all random smudges on white and withe patches on black manually. Then, if really needed (usually if it goes for coloring) I turn it to bitmap and then to grayscale again, to avoid anything else but black and white. I guess that does the trick for me, but I figured out everyone has it's own method for this, and if my explanation isn't that good, you may check one of the many tutorials on line for fixing b&w image.
