This the back of a Maiko (a Geisha apprentice). Maiko's can be in training for years until they are deemed fit to become full-fledged Geishas.

The 2 stripes of skin on the nape of the neck is usual. For formal occasions, such as the ceremony when a Maiko becomes a Geisha, their would be 3 lines of skin showing.
